If you need to cancel an appointment, please do so at least 48 hours
in advance. We reserve the right to charge a fee for missed
appointments

a list of items that may help you in preparing for your visit to our office.
Please let us know if you have questions or concerns that can further assist us in
making your visit productive, educational and comfortable for all!
Remembering
these items will help our office and you when visiting us for medical care.
Please bring with you, or be prepared with:

Your past medical history

Over-the-counter medications (when possible, bring the
actual medications)

A list of (or the actual) prescription medications you are
currently taking and the dosage instructions

Insurance information/card. Please confirm with us
that we accept your insurance coverage prior
to your visit.
