Most HMO's require referral to see any doctor other than your
primary physician. If you do not obtain a referral for a
visit with a specialist, you may not be seen. Please be
aware that referrals often require authorization your insurance
company. For this reason, we ask that you allow our
referral coordinator at least 3 business days to process all
referrals.
NOTE: We do not "back date" referrals.
For your convenience,
download our referral request form below.
Referral
request form Please note the
following additional important information about referrals:
-
It takes 3 days for a referral to be complete, unless it
needs to be authorized from the insurance company; then it may take
longer.
-
We do not fax referrals. It is the patient's
responsibility to get the referral to the specialist office.
-
Referrals need to be picked up between 8:30 a.m. and 4:00
p.m., unless other arrangements have been made.
-
Please note; the referral line voicemail does indicate how
long a referral takes to be processed, the hours of referral operation and
that we do not fax referrals.
-
For procedures and surgeries, the specialist needs to
request the referral and we will in turn send the referral directly to
them.
About
Your Insurance
We participate with many insurance companies. Often the insurance provider
directories can be outdated. Please confirm with us that we accept your
insurance coverage prior to your visit. There are many insurance companies
that have switched to a non-referral system. At the time of your visit, you
will be expected to pay any deductible and co-payment amounts. We stress to
you the importance of becoming familiar with your insurance coverage.
Please contact them with any questions or concerns regarding your coverage.
We will try to facilitate the best health care possible within your
insurance company system.
